Kiosk Mode

If a patient arrives for their appointment and has not started or finished their intake forms, you can activate Kiosk Mode to let the patient fill out their information on their mobile device or tablet. 

  1. Start by searching for a patient using the Search bar in the upper-left corner of the screen. Or, if you are on the Patients tab already (shown below), you can use the patient Search bar.

  2. Click on the patient's name to be brought to the Patient Details page.
  3. Scroll down. On the left side, select Complete Intake in the Send Intake box. 
  4. Use the Complete Intake drop-down to select the intake form you want to provide to the patient. You can click Kiosk Mode only after selecting the form you want.

  5. After clicking Kiosk Mode, a box with a QR code, an Intake form URL, and an Access Code will appear. These are all unique to the form and to the patient selected.

  6. Scan the QR code from the screen on your clinic's intake device, make sure the form opens, and hand the device to the patient to complete.

If the QR code or device isn't working, you can provide the patient with the Intake form URL and Access Code for them to fill out the form on their own mobile device or another in-clinic device that is specifically for patients.

What is a QR code?
QR codes are links to websites that can be scanned by the camera on your clinic’s tablet. It’s faster and easier than typing out an entire web address.

Apple Devices

Some devices, like recent iPhones or iPads, have QR code reading capabilities included in the camera application automatically.

  1. Open the Camera application.
  2. Direct the camera at the QR code.
  3. Scan the QR code and follow any on-screen prompts that direct you to the link. 

Android Devices

Recent Android devices utilizing Google Lens can follow the same steps outlined for Apple devices above; just open the camera and scan the QR code. 

For older Android devices, there are multiple free applications available in the Google Play store that can be downloaded, and paid apps that will not have advertisements in the application.

  1. Download the application of your choosing.
  2. Open the application from your Apps screen.
  3. Scan the QR code and follow any on-screen prompts that direct you to the link.
